CAPK WIC can help you maintain a healthy pregnancy and ensure a strong start for your child up to age 5 with:

CAPK WIC Provides:

  • Healthy Foods: WIC benefits to purchase healthy foods like bread, eggs, and milk to supply you and your family with essential nutrients like calcium, iron, and protein.
  • Nutrition Education: Registered Dietitians, Nutritionists, and trained professionals who can help inform, educate, and counsel you on the best nutrition for you and your growing family.
  • Breastfeeding Support: Personal, group and peer support from Lactation Educators to help you meet your breastfeeding goals.
  • Referral Services: Referrals to important and necessary community resources, which can help you and your family, find health care, child care, housing, etc.

How do I sign up?

Step 1: Contact us—fill out this form, or call us at 1-866-327-3074.

Step 2: Prescreening and Appointments: We will ask you questions over the phone to verify your eligibility and set up an appointment for you to come into one of our offices.

Step 3: Sign up for CAPK WIC: Each person who is enrolling in WIC must be present at the appointment. In addition, you will need to bring the following documents with you:

  • Identification for anyone who is enrolling (including children age birth to 5)
  • A document that verifies your current address
  • If applicable, a paycheck, tax return, or letter from your employer verifying your employment
  • If applicable, a document that verifies your pregnancy

Step 4: Meet with a Counselor—at your appointment, you will meet with one of our counselors who will:

  • Evaluate the nutrition needs and desires of each person enrolling
  • Assign a nutrition plan/food package to each person enrolling
  • Review WIC’s services and benefits
  • Teach you how to use your WIC benefits

Who can enroll?


Generally, WIC is available to:

  • low income pregnant, breastfeeding, and postpartum women;
  • Grandparents, dads, and foster parents can enroll!;
  • low-income parent or guardian who is the sole provider of children under age five who are at nutritional risk and who are at or below 185 percent of the federal poverty level. See income guidelines.
