Case Manager

Provides services of education and support to individuals and families with children ages birth to 5 years of age through home or community visitation. AA/AS degree in a health, social services, early childhood development or related field. Experience working with children 0-5 years of age. One year of case management experience. Two years’ experience working with underserved families of diverse cultures within the home and social services settings. Any equivalent combination of education and/or experience may be substituted on a year for year basis. Must be fingerprinted and have such records filed with the State Department of Social Services and Community Care Licensing. Must have access to reliable transportation during work hours. Possession of a valid California Driver’s License & State automobile insurance, and an acceptable driving record substantiated by a DMV printout. $16.75/hr. + benefits Open Until Filled
